You both have parts of it right. As I indicated earlier, I believe it's a TE-60B (or a TE-60). I'm not certain of the original intro date, but it was available with TS-40 switchers, but was also used into the early 70's in TS-51 switchers.

My earlier hasty answer should have said "Effects" not just wipe, because this device does more than just generate wipes. It's a keyer, as well.
Re: Bonus points: In lieu of model number I'm going to estimate the vintage as 1966-69... although my gut says 1965. And I would have loved to have had my 10-year old hands on one back then : )
